~Cancellation Policy~
In fairness to others and to us, 24 hours advanced notice is required for cancellation of an appointment or you will be charged in full for the time booked. Additional notice beyond what is required is greatly appreciated. Thank you for your consideration.
~Insurance~
Out of Network with Premera Effective 9/2/2011
We are only accepting L&I workers compensation and
PIP Auto Accident insurance at this time.
All insurance work must be medically necessary and curative. A valid prescription including frequency, duration and diagnosis is required for all insurance work! Palliative care is not covered (a treatment that makes you feel better but does not last or improve your overall condition). PIP Auto accident insurance billing and L&I work is possible depending on many factors including but not limited to your insurance carrier and plan. Additional forms as well as approval from your insurance company are also required prior to your first appointment. Insurance companies cover massage therapy when it is used to recover from an injury or condition. Following the frequency and duration listed on the prescription is important.
~Other Considerations~
Credit cards are not accepted please be prepared.
If you are taking precription medication, please confirm with your doctor that massage is not contraindicated.
Refrain from over the counter medications and alcohol prior to your session.
Please let us know ahead of time if you are pregnant.
Prepare for your bodywork session by hydrating yourself.
